Cats also have it made here at
Dundee Memorial. They'll
enjoy their own very special
"kitty condo" (shared with a
kitty friend from home if
another comes with them)
where they can lounge on
whichever level they choose.
We give them plenty of indoor
playtime in their romping
room where they'll find toys
and little amusements to
occupy them. Clean kitty
litter and fresh water are
provided at all times.

too. Whatever you
need, we'll provide. Do

how your pet is doing?
Want special
one-on-one attention
for your cat or dog?

favorite toy or
blanket? No problem!
We are pet-owners

We provide immaculate, comfy and
spacious single or double occupancy
kennels with privacy walls and skylights.
Plenty of clean, fresh water is provided at
all times.
If you have special treats, bring 'em on
in! Special concerns, med's, diet or needs?
Please be sure to include those in the
form below or by phone. While you're away
doggies will play in our safe, fenced romp
yard. At night your best friend will be
safely tucked in his or her own comfy,
private indoor state-of-the-art suite for
a good night's rest. We'll take care of
everything until you return.

As an authorized owner or agent of the above pet(s), I have read and understand the regulations. While boarding, I
give Dundee Memorial all rights to treat these pets as they deem necessary for any medical emergencies that may
arise. I understand that I accept full financial responsibility for these treatments and Dundee Memorial will make
every possible effort to contact me should an emergency arise. To further protect all pets, any boarding animal that is
found to have parasites such as worms, fleas or ticks, will be treated at the owner's expense. I agree to pay additional

any pet for any reason including health, physical condition,
aggressiveness or for the safety of the pet or staff.

You can help make check-in smoother by completing the form below. The information
you provide will be submitted directly to our office and we will NOT give, trade or
sell this information to any third party, ever! By answering these questions, you are
providing Dundee Memorial with important information for the protection of your
pets and other boarders in our care. If you indicate a special diet, please remember
to bring enough food for your pet for the duration of his or her stay, otherwise (s)he

A little boring stuff but important for the safety of all: To protect all pets, each boarding pet must have current vaccinations and an annual comprehensive
exam. If your pet is due for any vaccinations, we will be happy to have one of our doctor administer them with an exam. Please let us know if you will require
this when you make reservations. The standard vaccination fee applies. If you are visiting, you or your veterinarian may FAX your pet's current records to us
at (719)783-9088. In some instances, due to age or health, we may waive vaccination requirements. Wavers must be approved prior to arrival. Vaccination
requirements are listed below in the upper right corner of the Boarding Consent Form.
